Help with PESEL no.

I am soon to receive my Polish citizenship and Polish Passport. I have now been told that I have to apply for my PESEL no.

I live in Australia and don't speak Polish so am unsure how to fill it in. I also don't know how I am supposed to write in my PESEL no when I don't have one.

Does anyone know the process and the translation of the application form? Any help would be really appreciated. Thank you

by the way should I just wait to receive my papers before proceeding with filing in a PESEL form. Do I need any info from the citizenship papers to fill in the PESEL form?

I just filled this out for my son, ask me anything!

If you never had a PESEL number leave line 1 blank, and cross it out.


By the way in section VI, they want to know the legal reason why you need a PESEL number. The legal reason is "Rozdział 7a Art. 31a ust. 3 pkt 4"

So, you can answer this question as follows:

"PESEL jest mi potrzeby aby otrzymać paszport zgodnie z Rozdział 7a Art. 31a ust. 3 pkt 4"

In english this means: "I need a PESEL number to get a passport in accordance with.... and the actual law number"


For anyone who wants the law, here it is:

Rozdział 7a
Nadawanie numeru Powszechnego Elektronicznego Systemu Ewidencji Ludności (PESEL)


Art. 31a.
1. Numer Powszechnego Elektronicznego Systemu Ewidencji Ludności, zwany w niniejszej ustawie "numerem PESEL", jest to 11-cyfrowy, stały symbol numeryczny, jednoznacznie identyfikujący osobę fizyczną, w którym sześć pierwszych cyfr oznacza datę urodzenia (rok, miesiąc, dzień), kolejne cztery - liczbę porządkową i płeć osoby, a ostatnia jest cyfrą kontrolną służącą do komputerowej kontroli poprawności nadanego numeru ewidencyjnego.
2. (30) Numer PESEL nadaje minister właściwy do spraw wewnętrznych w formie czynności materialno-technicznej.
3. Numer PESEL nadaje się:
.......1) (31) obywatelom polskim zameldowanym na pobyt stały lub czasowy trwający ponad 3 miesiące, a także osobom ubiegającym się o wydanie dowodu osobistego;
.......2) (32) cudzoziemcom zameldowanym na pobyt stały lub czasowy trwający ponad 3 miesiące na podstawie art. 26 ustawy;
.......3) obywatelom polskim i cudzoziemcom, którzy podlegają na terytorium Rzeczypospolitej Polskiej ubezpieczeniom społecznym lub ubezpieczeniu zdrowotnemu, z wyłączeniem osób, którym nadano ten numer na podstawie pkt 1 i 2;
....... 4) obywatelom polskim zamieszkałym za granicą, ubiegającym się o wydanie paszportu.
4. Nadanie numeru PESEL następuje, w przypadku osób:
.......1) o których mowa w ust. 3 pkt 1 i 2 - na wniosek właściwego organu gminy;
.......2) o których mowa w ust. 3 pkt 3 - na wniosek płatnika składek ubezpieczeniowych;
.......3) o których mowa w ust. 3 pkt 4 - na wniosek polskiego konsula.

Art. 31b.
1. (33) Minister właściwy do spraw wewnętrznych może nadać numer PESEL osobom innym niż wymienione w art. 31a ust. 3 na ich pisemny wniosek, jeżeli odrębne przepisy przewidują potrzebę posiadania numeru PESEL przez te osoby.
2. (34) Osoby, o których mowa w ust. 1, składają umotywowany pisemny wniosek o nadanie numeru PESEL bezpośrednio do ministra właściwego do spraw wewnętrznych, wraz z odpisem skróconym aktu urodzenia, a w przypadku osób, które wstąpiły w związek małżeński - wraz z odpisem skróconym aktu małżeństwa opatrzonym adnotacją o aktualnie używanym nazwisku.
3. Wniosek o nadanie numeru PESEL powinien zawierać następujące dane:
.......1) nazwisko i imiona;
.......2) nazwisko rodowe;
.......3) imiona rodziców;
.......4) nazwisko rodowe matki;
.......5) płeć;
.......6) datę i miejsce urodzenia;
.......7) obywatelstwo;
.......8 ) adres i miejsce pobytu stałego lub czasowego;
.......9) rodzaj, serię i numer dokumentu tożsamości.
4. (35) Minister właściwy do spraw wewnętrznych może, na umotywowany pisemny wniosek organów, o których mowa w art. 31a ust. 4, i osób, o których mowa w art. 31b ust. 1, zmienić numer PESEL.
5. Numer PESEL podlega zmianie w przypadku:
.......1) sprostowania daty urodzenia osoby, dla której był nadany;
.......2) sprostowania oznaczenia lub zmiany płci osoby, dla której był nadany;
.......3) sporządzenia nowego aktu urodzenia stosownie do przepisów ustawy z dnia 29 września 1986 r. - Prawo o aktach stanu cywilnego (Dz. U. z 2004 r. Nr 161, poz. 1688);
.......4) stwierdzenia faktu wprowadzenia w błąd organu co do tożsamości osoby, dla której był nadany.
6. Przepisy ust. 2 i 3 stosuje się odpowiednio do zmiany numeru PESEL.

Art. 31c.
(36) Minister właściwy do spraw wewnętrznych powiadamia o nadaniu numeru PESEL organy, o których mowa w art. 31a ust. 4, oraz wnioskodawców, o których mowa w art. 31b ust. 1.

To apply for a PESEL, all you need is your Polish Registration of Birth, very similar to a Polish birth certificate. That's it, nothing, else, oh and the PESEL application form of course.

I haven't received my papers yet but the length of time it takes I thought I'd better get prepared. I'm from Australia so not sure if the paperwork is slightly different?

The consulate told me that they would give me the number when I come to apply for a passport.

When we visited the consulate to apply for my son's passport, the consulate checked some sort of internal system directly in Warsaw to retrieve his actual PESEL number electronically. We copied it down onto the passport application, and done!

Thanks everyone..In the end long story but since I had changed my name I needed to get a new Polish birth certificate and once that is done the lawyer will apply for the PESEL on my behalf...2 months I've been told to register my name change and then the Pesel and then probably about 18months? for my very first passport to be issued.

Did you mail in your PESEL application before going to the consulate or did you make 2 separate trips?

Well in the end after I got the new Polish Birth Certificate I was then told to fill in
the form to get my Pesel no. So the lawyer filled it in on my behalf, although one of my Polish friends did give me a bit of guidance as to how to fill it in, but by that stage I was so fed up I didn't want to risk making a mistake. So I waited and then it was sent to me. I was under the impression that I needed to have it before I applied for my passport. Because whenI went in with all my documents she asked me for my Pesel no, so I can't be sure she would have looked in the system to get it. So yes I did mail away for it as I didn't want to fly to Sydney twice.

Sad thing is I started this process in 2005 and I only got my passport in August 2010. The struggle of it all, the cost. As it has taken so long I'm not sure when I'll be able to actually use it. The biggest mistake was to have used Icharsky law firm in Israel. Had I have known I would of started with a Polish Lawyer. 2 years was wasted.All the best in your adventure. I actually faxed in the application for the PESEL. They emailed me a few days later to tell me that it was granted, and when I showed up at the consulate for the passport application, they looked up the PESEL number remotely in the Warsaw source system right in front of me. They wrote down the number into the passport application, and then proceeded to process it.
