The EEA Family Permit is either for you and your wife travelling together, or you joining her here. You cannot travel alone with this permit on your passport.

When did you move to Poland and are you working there? It may be useful to explore your residency options while in Poland, unless you just got there.

Polish residency requires 5 years stay in Poland (with allowed trips out of the country no more than 6 months in a year - pretty standard for a lot of countries). And while in Poland you must be exercising treaty rights, i.e. studying, working, being self-sufficient or self-employed.

So at this point your best bet is an EEA family permit, but as I mentioned earlier, you need to travel with your wife or meet her here in the UK.
