My personal experience with mis-matched names is when I applied for a Canadian passport, they gave it to me, after A LOT of convincing. They really did not want to issue it, but they made me promise (verbally) to get it fixed next time. They made a note in their computer about the issue, and that I would probably be denied my next passport if I did not have it fixed by then.
I though about it, and I decided to get things straightened out before I needed the next passport, and before I started the Polish citizenship process.
Poland may not be as strict. But who's to say that it will be a Polish official that makes a stink about it.
Imagine you are stopped at customs or at airport security for a completely random check. They decide to run through your records, and find an undocumented mis-match. Let your imagination run wild for what happens next....strip search? Getting detained and missing your flight? Being kidnapped to Guatanamo? Who knows...
Why take the risk, and why have this hanging over you every time you try to use the passport? I would get it fixed sooner than later.
