✎ EN Birth certificate for parent

    My mother was born in Lublin in 1942. I got in touch with the archives in Lublin using 2 last names in relation to the individual in question. Their response was negative on both. Some scanned documents I received from the ITS shows a last name of Slyszko for my mother.

    Since I do not know who her biological father is and a failed search, I am thinking of filing a birth certificate for her since it can not be found or is assumed destroyed.

    Who would I get in touch with to find out how to file this? She was adopted after her mother got remarried and a declaration for a certificate was requested and the last name of the 2nd husband was used.
